All participants in all Scouting activities must complete Part A and Part B. Give the completed forms to your unit leader.
This applies to all activities, day camps, local tours, and weekend camping trips less than 72 hours and must be updated annually.
Part C – A pre-participation physical certification is needed for resident, tour, or trek camps or for a scouting event of more than 72 hours. The exam needs to be completed by a certified and licensed physician (MD, DO), nurse practitioner, or physicians assistant.
